If a Cash-Pay Coupon is eligible for your use, the offer will be available at checkout.
In such case, by applying for that Cash-Pay Coupon offer, you acknowledge and agree that:
• You elect to pay for such prescription medication using all cash, meaning that you elect to pay for the medication outside of any applicable insurance benefits, including but not limited to any state or federal healthcare program, such as Medicaid, Medicare, Medicare Part D, Medigap, Department of Defense, Veterans Affairs, TRICARE®/CHAMPUS, or Employer Group Waiver Plans;
• You will not seek reimbursement from any healthcare program or insurance for the cost of this medication; and
• You will not attempt to apply the cost of this medication toward any deductible or out-of-pocket maximum.
